This Telehealth Consent describes important information about receiving healthcare services via telehealth, including asynchronous messaging, phone, and/or video, as offered through the Viril platform.
What Telehealth Is
Telehealth involves the use of electronic communications and information technologies to deliver clinical services when you and the provider are not in the same physical location. Your interactions may include written questionnaires, secure messages, phone calls, and/or video visits.
Expected Benefits
- Convenient access to care without an in-person visit.
- Timely review of your health information and concerns.
- Ability to receive evaluation, treatment recommendations, and prescriptions where clinically appropriate and legally permitted.
Risks
- Information you provide may be incomplete or inaccurate, which may affect clinical decision-making.
- Telehealth may not be appropriate for all conditions, including urgent or emergent issues.
- There may be technical failures (for example, poor connectivity) that could delay or interrupt care.
- Despite reasonable safeguards, there is a risk that electronic communications could be accessed by unauthorized persons.
- A provider may determine that telehealth is not appropriate and may recommend in-person evaluation or emergency services.
Alternatives
Alternatives to telehealth include in-person evaluation with a healthcare provider. You have the right to seek care outside of the Viril platform at any time.
Prescriptions and Follow-Up
If a provider determines treatment is appropriate, the provider may prescribe medication and/or recommend labs, follow-up visits, or in- person care. Not all conditions can be treated via telehealth, and not all users are eligible for prescriptions.
Emergencies
Telehealth is not for emergencies. If you think you may have a medical emergency, call 911 immediately or go to the nearest emergency room.
Privacy and Security
Viril uses safeguards designed to protect information. Your provider may document your care and communications in a medical record. For more information about how we handle information through the platform, see our Privacy Policy and other notices.
Your Consent
By proceeding with telehealth services through the platform (including submitting an intake), you acknowledge and agree that:
- You understand the nature of telehealth and its limitations.
- You consent to receiving healthcare services via telehealth where legally permitted.
- You agree to provide accurate and complete information to the best of your ability.
- You understand you can stop using telehealth services at any time and seek in-person care.
